MEDIA ADVISORY: Clinton Foundation, The San Diego Foundation, County of San Diego Announce Bold Action Steps to Improve Health and Well-Being of San Diego Children & Families
Strong Families, Thriving Communities Coalition of community leaders will meet to launch action steps in new “Blueprint for Action”
San Diego, CA – On Monday, March 12, the Clinton Foundation, The San Diego Foundation, and the County of San Diego will convene key thought leaders and local stakeholders to launch a detailed plan to improve the health and well-being of children and families in the San Diego region. This “Strong Families, Thriving Communities Blueprint for Action” focuses on increasing equity and effectiveness for families who interact with San Diego’s child welfare and juvenile justice systems by aligning policies, practices, and resources.
The Blueprint for Action model is being implemented by the Clinton Foundation Health Matters Initiative (CHMI). The model is currently adopted in areas across the country, including Adams County, Mississippi; Central Arkansas; Knox County, Illinois; Northeast Florida; and Greater Houston, Texas. The Coachella Valley of California recently completed its five-year Blueprint and is continuing to work to improve health and wellness. See more information about the results here and here.
In March 2017, CHMI, The San Diego Foundation, and the County of San Diego announced that San Diego would be the seventh community to join CHMI’s Community Health Transformation portfolio.
For the past year, representatives from CHMI, The San Diego Foundation, and the County of San Diego engaged with more than 100 community partners – the Strong Families, Thriving Communities Coalition – in a process that identified key challenges and solutions through bold action steps for children and families involved in the child welfare and juvenile justice systems.

Our programs are designed to make a real difference today while serving as proven models for tomorrow. The goal of every effort is to use available resources to get better results faster – at the lowest possible cost.
We firmly believe that when diverse groups of people bring resources together in the spirit of true cooperation, transformative ideas will emerge to drive life-changing action.
