We work with regions disproportionately impacted by chronic disease to improve health outcomes and close gaps in health disparities. To guide our community health transformation work, we use the County Health Rankings Model. Our current focus communities include: Adams County, Mississippi; The Coachella Valley, California; Central Arkansas (Little Rock); Northeast Florida, Florida (Jacksonville); Greater Houston, Texas; and Knox County, Illinois.
In each region, we serve as a neutral convener to pull together key stakeholders across sectors to create a local Blueprint for Action based on the unique health indicators of each community.
Specifically, we work with our focus regions to improve health outcomes and close gaps in health disparities by:
- Synthesizing a set of unique community health indicators to determine areas of greatest need,
- Convening key stakeholders from across sectors for an initial blueprinting process to set local priorities,
- Facilitating solutions across sectors to implement priority actions included in a local Blueprint for Action, and
- Leveraging national, regional, and local resources that will help advance priority actions in the local Blueprint for Action.
